Empowering New Beginnings and Building Stronger Futures
Reintegration Services
Founded in June 2022, our organization is dedicated to strengthening the San Benito County and Monterey County by providing compassionate support and practical solutions for those facing life’s toughest challenges. We serve Medi-Cal members, veterans, unhoused individuals, justice-involved, and other vulnerable populations who too often fall through the cracks of fragmented systems of care.
As of October 2025, we’re proud to share two major milestones:
✅ We’ve established a new office space to expand our in-person services.
✅ We’re now an approved Medi-Cal Provider with California’s Department of Health Care Services (DHCS)—a critical step toward becoming an approved Medi-Cal Provider with CCAH offering even more impactful programs through the Central California Alliance for Health. "We have submitted our applications for approval with the Central California Alliance for Health to offer enhanced services like Enhanced Care Management (ECM) and Community Supports (CS) and we are awaiting approval."
