Founded in June 2022, our organization is dedicated to strengthening the San Benito County and Monterey County by providing compassionate support and practical solutions for those facing life’s toughest challenges. We serve veterans, unhoused individuals, justice-involved, other vulnerable populations who too often fall through the cracks of fragmented systems of care, and we will be able to serve Medi-Cal members if and when we become approved by CCAH.
As of October 2025, we’re proud to share two major milestones:
✅ We’ve established a new office space to expand our in-person services.
✅ We’re now an approved Medi-Cal Provider with California’s Department of Health Care Services (DHCS)—a critical step toward becoming an approved Medi-Cal Provider with CCAH offering even more impactful programs through the Central California Alliance for Health.
"Our applications for approval with the Central California Alliance for Health to offer enhanced services like Enhanced Care Management (ECM) and Community Supports (CS) we are put on hold due to budgetary constraints and new language changes brought on by the federal cuts to health care funding. In 2026 we will apply to become a Provider of (CHW) Community Health Worker Services with CCAH”!
Our nonprofit was founded by L. Mitchell, RN, and D. Bouie in June 2022, with the aim of helping people in our community who are underserved and in need of help, from the unhoused, the previously incarcerated, to all others who are at-risk!
We have been a client of CSUMB’s IIED Program since 2023. In October 2025 our nonprofit became approved Medi-Cal Providers with the (DHCS) Department of Health Care Services! If our upcoming applications are approved to become Providers with (CCAH) Central California Alliance for Health, to provide CHW services we will be able to expand the services we currently provide. As always, we will be seeking any help in acquiring building/office space for a future Shelter and an (LTORR) Transitional Reentry Program for the Justice Involved, and others who are substance abusers, unhoused, or on probation, that local and State Law Enforcement contractually sends to us to attend our future, various self-help programs!

How We Help
Our volunteer Exec. Director Daryl Bouie and a small team of volunteers meets people where they are, by referral—out in the community—and in-office—offering the tools and support needed to achieve stability and independence.
We provide:

Life-skills training, including financial literacy and health education
Self-help and peer support counseling
Donation and resource assistance for the unhoused and underserved
Guidance and empowerment for individuals transitioning from incarceration
