Founded in June 2022, by Registered Nurse L. Mitchell and D. Bouie, our organization is dedicated to strengthening Monterey and San Benito Counties. We provide compassionate support and practical solutions for veterans, unhoused individuals, the justice-involved, and vulnerable neighbors who too often fall through the cracks of fragmented care systems.
Major Milestone: Partnering with United Way
We are proud to announce a new Partnership Agreement with The United Way of Monterey County. Together, we are expanding access to essential services:
✅ Financial Empowerment: Access professional Financial Coaching through the United Way’s "Smart Referral Network”.
✅ CHW Services: Our Community Health Workers are now available at the Community Impact Center (232 Monterey Street, Salinas).
✅ 2-1-1 Integration: Find our full list of resources and support through the 2-1-1 information system.
Our Growth & Impact
As of 2025-2026, we have reached several critical milestones to better serve you:
Expanded Presence: We have opened a new office space to increase our capacity for in-person support and case management.
State-Certified Care: We are an approved Medi-Cal Provider with the California Department of Health Care Services (DHCS).
Enhanced Benefits: Starting in May 2026, we will begin offering CalAIM CHW Benefit Services in partnership with the Central California Alliance for Health (CCAH).
Our Mission: To ensure no one in our community has to navigate their toughest challenges alone.
The Heart of Our Work
Founded in June 2022 by L. Mitchell, RN, and D. Bouie, our nonprofit was born from a shared vision: to serve the underserved and provide a lifeline to those at-risk in our community. With a foundation in healthcare and a passion for social advocacy, our founders set out to catch those who too often fall through the cracks of fragmented systems.
Since 2023, we have been a proud client of the CSUMB Institute for Innovation and Economic Development (IIED), a partnership that has helped us scale our impact and refine our mission to better serve San Benito and Monterey Counties.

Medi-Cal Integration
In October 2025, we reached a pivotal milestone: becoming an approved Medi-Cal Provider with the California Department of Health Care Services (DHCS).
Starting in May 2026, we will be utilizing the Pear Suite platform to deliver high-quality Community Health Worker (CHW) services. This allows us to provide professional, data-driven support to:

Anyone seeking to improve their situation through dedicated case management.
Our Goals for the Future
We are not just looking at where our community is today, but where it needs to be tomorrow. Our long-term goal is to bridge the gap between crisis and stability through specialized housing and reentry programs.
The LTORR Project or other Projects
We are actively seeking support and funding to establish a dedicated office space and a Long-Term Office & Residential Reentry (LTORR) Program, or other Projects. This facility will serve:
The Justice-Involved and those on probation.
Individuals navigating recovery from substance use.
Our unhoused neighbors in need of a safe, stable environment.
By partnering with local and state law enforcement, we aim to provide a transformative space for self-help programs and restorative justice.
How We Help
Bridging the Gap to Stability and Health
Starting in May 2026, our leadership and team are taking our mission directly into the community. Led by our Executive Director and State Certified CHW and PSS, Daryl Bouie, we are proud to offer on-site Financial Coaching and Community Health Worker (CHW) Services in partnership with the United Way of Monterey County.
By leveraging the 2-1-1 Referral system and the Smart Referral Network, Daryl and our dedicated CHW Interns "meet people where they are"—whether in our office or out in the community—providing the essential tools needed to achieve lasting health and financial independence.
